In Stahl's transporter chapter, SERT and the vesicular monoamine transporter VMAT2 do not work in the same location. SERT is associated with reuptake across the cell membrane, whereas VMAT2 loads monoamines into intracellular vesicles. Being transporters does not make their direction or destination identical.
A substance binding to a receptor is a different process from transport. The textbook's agonist spectrum distinguishes full agonists, partial agonists, neutral antagonists and inverse agonists through receptor response. Partial agonist does not mean half a tablet or half the effect in every person.
Our original worksheet asks 'where?' before 'what does it do?' Cell membrane, vesicle and receptor are separate cards. They teach mechanistic language, not the cause of a disorder or a suitable medicine for a person. A simplified synapse diagram cannot replace evaluation of clinical benefits and adverse effects.
Apply it: fictional worksheet
Fictional matching: match 'reuptake across the cell membrane', 'loading a vesicle' and 'altering receptor response' to SERT, VMAT2 and the agonist spectrum. Explain in one sentence why the diagram cannot select medication for an individual.
Example reasoning and its limit
SERT: reuptake. VMAT2: vesicular loading. Agonist spectrum: receptor response. The scheme concerns mechanisms and lacks the information needed to judge clinical suitability.
